Fill out the online intake form to get connected to a doctor. Our service is HIPAA-compliant, so all personal and medical information is completely confidential between you and your doctor. You will NOT be charged if the doctor believes you do not qualify for a medical card.

The next step is to visit with a doctor using any video-capable device from the comfort of your home. Our doctors will work with you to create a treatment plan and provide you with all the documentation necessary to get your medical card in the State of Rhode Island.

Once you have been certified by one of our doctors, they will complete the Practitioner's form for you. You will then need to apply with the State of Rhode Island using the patient form and pay the $50 state application fee. Fill out the patient form, print it out, and mail it in with the original practitioner’s form. The doctor will answer any questions you have about the process. If the Health Department accepts your application, they’ll ask you to schedule an in-person meeting to get your card.
